Filling Out an Accident Report Form

It is essential that any information about an accident within the workplace is recorded. Sometimes this can seem tedious but accident reporting can help identify trends, control health and safety risks and could prove useful if any investigations take place.

The accident book can be filled in by anyone; however any first aiders will need to be present to ensure that the information entered onto the form is accurate. The following list comprises the information which may need to be collected:

  • Date , time and location of the incident
  • Details of any injuries sustained
  • Information about the first aid provided
  • Name and signature of first aider or person dealing with incident
  • Signature of casualty or representative if unable to sign

Early Years Settings

Accidents within Early Years Settings will require a slightly modified form. Additional pieces of information that are required include information about what has happened to the child after the event. Alongside information about the Parent / Carer and the time of which they were contacted.

The best time to complete the form is after the incident when the events are still fresh in people's mind. Although the treatment and clearing up process should take priority over paperwork.

The reporting process should be both honest and accurate. First Aiders should never be pressured to write anything other than what has happened and are protected in the sense that they were only trying to help in a challenging situation.
